NEW YORK (Reuters Health) - Using volumetric laser endomicroscopy (VLE) to mark suspicious areas could improve neoplasia detection in Barrett’s esophagus (BE), according to a first-in-human feasibility study.

VLE provides a circumferential scan of the entire distal esophagus up to 3 mm in depth, and one device includes a laser marking system capable of applying superficial cautery marks on the esophageal mucosa to identify tissue that can be biopsied later using white light endoscopy (WLE).

Dr. J. J. Bergman from Academic Medical Center Amsterdam in the Netherlands and colleagues evaluated the visibility and positional accuracy of laser marks in different esophageal tissue types on WLE and VLE in a feasibility study of 16 BE patients.

The mean total time of all endoscopic procedures was 56 minutes, including 22 minutes for the VLE imaging and laser marking procedures, the team reports in Gastrointestinal Endoscopy, online February 1.

Overall, 97% of the 222 total laser marks were visible on WLE, with a positional accuracy of 86%.

Eighteen areas in five patients were identified as suspicious for neoplasia and successfully targeted by 36 laser marks. A third of these targeted areas showed high-grade dysplasia or esophageal adenocarcinoma on histology.

There were no adverse events.

“In conclusion, this first-in-man study of the VLE-guided laser marking system showed promising findings,” the researchers conclude. “The vast majority of the laser marks were visible on WLE and VLE and accurately positioned. Targeting VLE areas of interest proved to be highly successful. VLE-guided laser marking may improve the detection and delineation of Barrett’s neoplasia in the future.”
